At FPM we are always keen to celebrate the amazing work Primary Care teams do for their patients.
That’s why we’re starting a new feature that showcases the fantastic things GP Practices are doing in their communities and the services they’re particularly proud of.
For our first feature, we spoke to Business Manager Mark Donaldson about the brilliant work of Oakwood Lane Medical Practice’s Health Champions, the challenges of caring for patients in an area with high levels of deprivation and taking a continuous improvement approach.

What is the main demographic of your practice?
The level of deprivation within the practice population is a challenge and is rated as Level Two on the scale, where Level One represents the highest level of deprivation and Level Ten marks the lowest.
We have a diverse patient population with many ethnicities, cultures and heritages represented. We have marginally more females than males, and approximately 12% of our patients are over the age of 65.

We’d love to hear about how you engage with your local community?
We engage with our patients and the local community in many of the traditional ways, but the primary way is through the Oakwood Lane Practice Health Champions.
Supported by the practice, the Practice Health Champions directly engage with our patients, other patient groups and local health and social care providers, using methods including:
- A set of weekly activities (see the next question for more information)
- Newsletters, leaflets and dedicated in-practice displays
- Patient surveys and feedback
- Engagement events such as health promotion fairs, seasonal fairs and fund-raising events
- Regularly scheduled meetings
Which services that you offer patients are you particularly proud of?
Counselling service – We offer patients in rapid need of psychological therapies access to an in-house counsellor, so that they can receive a timely assessment from an appropriately trained professional.
Proactive care home visits – We offer patients in care homes a weekly visit led by an Advanced Nurse Practitioner. This facilitates effective monitoring and assessment, as well as meaningful care planning.
Patient-led services – We offer a wide range of services organised and delivered by the Oakwood Lane Practice Health Champions. Click here to see a short video outlining the services they currently offer.

In addition to the services above, we also host a large number of services. Our patients can access all of these, some of which are delivered at our practice exclusively for our patients:
- Female Genital Mutilation clinics
- Specialist Domestic Violence support clinics
- ‘Additional’ trauma based women only counselling service
- Community Midwifery clinics
- Consultant-led Memory Assessment clinics
- Specialist Chronic Pain Management clinics
- Specialist Bowel Cancer Clinics
